Dog Boarding Agreement

*ATTENTION* We ask that you please be on time. Drop offs/Pickups are BY APPT ONLY between the hours of 8-10 a.m. and 4-6 p.m. (Sundays 8-10 a.m.) The kennel is not always open for pickups/dropoffs outside of your appointment time. The kennel is closed from 10 a.m.-4 p.m.

THIS DOG BOARDING AGREEMENT (this “agreement”) is made and entered into by and between The Pet Companion in West Virginia State, and the person identified as the “dog owner” below. This agreement applies to all visits by your dog to The Pet Companion .

Services

The Pet Companion provides dog boarding services BY APPOINTMENT ONLY. Dog owner hereby engages The Pet Companion to provide the services for his or her dog(s) as set forth on the information sheet provided to the owner, the terms and conditions set forth in this agreement and in any supplemental dog information or reservation forms provided in connection with this agreement by The Pet Companion . 

Payment

Owner agrees to pay all fees for boarding, veterinary care or other services owed on the day of pick up. Rates are per calendar day regardless of time of drop off or pick up. A $35 fee for all returned checks. 6% sales tax is added. At this time credit cards are not accepted. Check, paypal, venmo or cash (discount)

Reservation/Cancellation

A $100 deposit is required to secure a reservation. Deposits are due within 24 hours of requesting a reservation. Deposits are refundable if cancelled at least 7 days prior to your drop off appt. 

OWNER IS RESPONSIBLE FOR PAYING FOR ALL DAYS RESERVED.

Health and Vaccinations

Dog must be up to date on the following vaccinations: Rabies, Dhlpp and Bordetella. Owner represents to The Pet Companion that to the owner’s knowledge your dog has not been exposed to any contagious diseases within a thirty day period prior to check-in.

The Pet Companion reserves the right to refuse to accept a dog at check-in for any reason, including without limit, if it appears the dog is sick, injured, in pain or that its behavior could jeopardize the health or safety of other dogs or our staff.

Owner acknowledges that dog is current on all required vaccinations. Owner agrees to provide documents confirming such vaccinations as requested by The Pet Companion prior to any entry into Kennel. Owner understands that all dogs boarded at The Pet Companion are fully vaccinated. However, it is still possible for your dog to become ill even if vaccinated. Owner understands that this is not due to any circumstance or condition at The Pet Companion and agrees to not hold The Pet Companion liable in the event your dog becomes ill during or after his /her stay. If at any time your dog is found to have fleas or ticks, The Pet Companion may provide the appropriate flea or tick removal treatment, and owner authorizes The Pet Companion to provide such service at owners expense.

Emergency Medical Treatment

Owner acknowledges that, in the unlikely event your dog becomes ill or injured, or if your dog has a pre-existing condition which is aggravated by its stay and requires professional attention, The Pet Companion will attempt to notify you or your emergency contact at the phone numbers you provided. The Pet Companion, at its sole discretion, may engage the services of a 24-hour emergency veterinary clinic or a veterinarian of our choice and/or administer medicine or give other necessary attention to your dog, and you authorize The Pet Companion to provide any such service at your additional expense. In cases we believe to be critical, we may take your dog to the veterinarian first before trying to contact you. If you refuse medical treatment for your dog, The Pet Companion will make your dog as comfortable as possible until picked up by owner or owners emergency contact. If we cannot reach you or your emergency contact, The Pet Companion will make healthcare decisions for owners dog based on the recommendations of available professionals.

In the unfortunate event that owners dog passes away at The Pet Companion, The Pet Companion will follow owners instructions on what to do with dog until owner returns. 

Emergency or Natural Disaster

In case of emergency or natural disaster, every effort will be made to contact you or your emergency contact to retrieve your dog. Owner agrees that The Pet Companion, at its sole discretion is authorized to transport, and/or make temporary alternative arrangements to house and care for your dog until you or your emergency contact can retrieve your dog. 

Group Activity

Owner agrees that if their dog is scheduled to interact with other dogs, that every effort will be made to ensure the safety of our guests. Owner acknowledges that, in the unlikely event owner’s dog is injured by another dog, you release The Pet Companion from any liability for such injury. If your dog injures another dog, you will be solely responsible for any injury to the other dog(s) as well as your own dog, and you release The Pet Companion from any liability for such injury.

Dog Behavior

Owner acknowledges that dog has never bitten or shown any agressive behavior towards humans or other dogs.

Owner is responsible for cost of replacing property of The Pet Companion that owner’s dog destroys or damages, including but not limited to beds, blankets, fencing and doors. Replacement Cost $15 for bedding; $100 for cots/toddler beds; doors, cables, etc. charge is full replacement cost.

Owner agrees that if dog becomes aggressive, destructive or disruptive, or acts in any manner that puts The Pet Companion and our representatives and employees in any danger The Pet Companion will attempt to notify the owner or their emergency contact to make other arrangements for your dog. In the event that owner or emergency contact are unavailable, The Pet Companion, at its sole discretion, may make other arrangements at an alternative boarding facility chosen by The Pet Companion. Owner will be billed and responsible for any fees incurred at both The Pet Companion and the alternative boarding facility.

Owner acknowledges that dogs that are kenneled together are from the same household. Owner assumes all responsibility for any aggressive behavior, fighting or injury that one dog may inflict upon another while they are together. The Pet Companion reserves the right to separate any dogs who are boarding together at our discretion for the dog’s own safety. If your dogs are not compatible boarding in the same kennel unit, owner will be billed and responsible for the cost of more than one kennel.

Photos and Video Release

Owner agrees to allow The Pet Companion to use his or her dog’s name and any images or likeness of his or her dog taken while dog is staying at The Pet Companion, in any form or format, for use, at any time, in any media marketing, advertising, illustration, trade or promotional materials.

Personal Property

Owner agrees that The Pet Companion will not be responsible for any lost stolen or damaged personal property belonging to either the owner or the dog. Owner also understands and agrees that the dog’s collar may be removed in the kennel and play areas to prevent injury to the dog. No choke collars will be left on dog.

Abandonment

Owner understands and agrees that if his or her dog is not picked up within 15 calendar days after the scheduled pick up date, or owner refuses to pay agreed upon charges, the dog shall be deemed to be abandoned.

Owner agrees to pay for attorney fees incurred by The Pet Companion in the collection of any boarding, hospital or other charges incurred by owner.

Although dogs will be supervised while in exercise yard, The Pet Companion, LLC is not responsible for dogs that dig out, climb or jump fence and escape

Owner has read this consent, understands and agrees to its terms.
